The two forms to the right are needed to register for Jimmy Weinert Training Facility. In order to view and print these forms, you will need Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader Installed, you can download the latest version by clicking here.
**NOTE** – If a rider is under 18, we must have a notarized medical form giving the training facility permission to care for them even if the parent is coming with them. If the parent is temporarily absent during their stay, then we need to be able to care for the rider if they become injured and need medical assistance.
